EMPLOYER MATCHING

for donations & volunteer hours

 

The CHAP Band THANKS you for your contribution and volunteer hours!
Your gift is tax deductible and could be matched dollar for dollar by your employer. Many employers offer matching gift programs and will match charitable contributions AND volunteer hours for their employees, retirees and/or employees’ spouses.

1.    FIND OUT

To find out if your company has a matching program, search your workplace intranet or contact your employer’s human resources department/HR, Employee Experience, Community Relations or Benefits departments and ask if your company matches employee donations and/or volunteer hours. If so, ask about the process and paperwork needed to get donations and volunteer hours matched. Be sure to TRACK your volunteer hours.

2.    FILL OUT

Complete any necessary paperwork/forms. Print or request your employer’s matching gift form OR submit via your workplace intranet. Fill out the Donor (employee) section. The Recipient section will be completed by WBP.

3.    SEND

Mail completed form to below address and WBP will take care of the rest.  The employer will send the donation directly to us. If you’d like Patron Recognition/Benefits, please also copy fundraising@westlakeband.org with matching documentation.
